Physical Therapist (PT) – Home Health
Position Details
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- Productivity Requirement: 30 points per week
- Mileage Reimbursement: $0.45 per mile
- Coverage Area: Pueblo, Fremont, Crowley, Custer, El Paso, Otero, Bent, Huerfano, and Las Animas Counties, CO
- License Requirement: Doctor of Physical Therapy (DPT) degree with an active Physical Therapist license
Preferred Qualifications
- Home Health experience preferred
- Experience with PointCare software preferred
- Strong clinical assessment and documentation skills
- Ability to work independently in a field-based environment
- Excellent communication and patient education skills
Position Summary
The Physical Therapist (PT) is responsible for providing skilled physical therapy services to patients in the home health setting. The PT evaluates patients, develops individualized treatment plans, implements therapeutic interventions, and collaborates with physicians and interdisciplinary team members to maximize patient independence, mobility, and overall quality of life.
Key Responsibilities
- Conduct comprehensive physical therapy evaluations and assessments based on physician orders and referrals.
- Review patient diagnoses, medical history, precautions, and contraindications to develop appropriate treatment plans.
- Establish measurable goals and individualized treatment programs to improve functional outcomes.
- Provide evidence-based physical therapy interventions to enhance strength, mobility, balance, endurance, and safety.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Communicate patient status, progress, and recommendations with physicians, caregivers, and interdisciplinary team members.
- Recommend and assist with obtaining adaptive equipment and assistive devices to improve patient function and safety.
- Complete and submit all evaluation, treatment, billing, and clinical documentation accurately and within required timelines.
- Maintain compliance with company policies, HIPAA regulations, state and federal guidelines, and professional practice standards.
- Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state practice regulations.
- Participate in staff meetings, training programs, and continuing education opportunities to maintain and enhance clinical competency.
